Create your account in four easy steps:

  1. Select Enrollee/Adult Dependent from the user type drop-down list.
  2. Enter your personal information (first and last name, enrollee ID and date of birth). To locate your enrollee ID number, review the welcome letter or email you received from Delta Dental. It might be called "member ID" or "subscriber ID," depending on your plan.
  3. If you can't find your enrollee ID number, enter your Social Security number — with no dashes or spaces — as an alternative.
  4. Create your username and password and enter your email address. You will also be asked to choose a challenge question and answer in case you ever forget your password.
  5. Now you can log in to your account from our website.

Log in to your online account (or create an account if you don't already have one) to check the status of your plan — active or inactive — and the start and end dates. If your plan is active, you can view the status of your dependents on the Family Members Eligibility Status List. Dependents who are currently covered under your plan are listed as active. If not, they're listed as inactive.
